Dear LAM Community,
My name is Laura Bowers, and I was diagnosed with LAM on December 23rd of 2013. Currently, I am a Pharmacy student in my 4th and final year at the UNC Eshelman School of Pharmacy. This year I am on a research rotation with my LAM Clinic physician, Dr. Charlie Strange, at the Medical University of South Carolina.
This letter is an invitation to participate in a research survey. The purpose of the research is to answer important questions about medication use and quality of life in LAM patients who are considering or who have undergone lung transplantation.
It should take approximately 20 minutes to complete the confidential questionnaire if you are considering lung transplantation and 30 minutes if you have undergone lung transplantation. You may skip most questions for which you do not know the answer or do not wish to answer.
Taking part in the research study is voluntary. Your completion of the questionnaire implies your voluntary consent to participate in this research study. The Medical University of South Carolina (MUSC) will not ask any identifiable or personal information in this survey. You will not be contacted by researchers now or in the future.
Summaries of findings and information gathered from this survey will be shared with LAM patients through The LAM Foundation newsletters and website after publication in a medical journal.
